I was going to say small-displacement straight 8, ala Alfa Romeo, but the spacing is tight, even for one of those, so that leads me to a 4cyl with two exhaust valves that each have their own port... I am reminded of the Lancia ECV, which used two turbos and complex exhaust valveing/routing to channel gasses to each turbo depending on flow rates and requested boost, it had twin exhaust ports, one for each exhaust valve, but that header is wrong for that use and the likelyhood of anyone one [anywhere] having one is very low... As far as automotive engines that have that setup, I'm not familiar... Perhaps it's for a motorcycle? The pipes do look quite small...

It’s a modded (shortened) manifold from a 406 Mi16 to fit in a 205, made by PR Developments (the ones who did a twin V6 205 and a supercharged V6 205 GTI) :

The bolt pattern match a XU9J4 head with 9 studs :

A 205 swapped with a 405 Mi16 engine, cams and ITBs, not sure if it has the specific exhaust manifold of the topic :
